What is this medicine?

Otrivin Nasal Spray is an anti-swelling spray, with an active ingredient - Xydometazolin, which acts on adrenergic receptors. Xylometazoline in nasal use causes narrowing of the blood vessels of the nasal mucosa and adjacent areas of the nasopharynx, thus eliminating swelling and hyperemia of the nasal mucosa and nasopharynx, and also reduces the associated increased mucus secretion and facilitates the removal of blocked secretions from the nose. So it is indicated for:

  • Symptomatic treatment of nasal congestion with colds, hay fever, with other allergic rhinitis, sinusitis.
  • To facilitate the outflow of secret in diseases of the appendage sinuses of the nose.
  • Auxiliary therapy in cases of otitis mean (to eliminate swelling of the mucous membrane).
  • To facilitate the rinoscopy.

What are the possible contraindications?

How should I use this medicine?

The drug is used by adults and children aged 12 years and over 1 injection into each nasal passage up to 3 times a day. Use no more than 3 times in each nasal passage per day. The interval between applications should be at least 8-10 hours. The duration of treatment depends on the course of the disease and should not exceed 5 days without a break. Do not cut off the spray sprayer. Spray is visual, dosed immediately ready for use. The dosed spray provides dosage accuracy and proper distribution of the solution on the surface of the nasal mucosa. Each injection is administered 0.14 ml of a solution corresponding to 0.14 mg of xylometazolein. Before the first use, prepare a sprayer by pressing it 4 times. The sprayer will then be ready to use throughout the treatment period. If the spray is not sprayed while pressing the sprayer or the drug has not been used for more than 7 days, the spray spray should be prepared again with 4 spraying in the air. Interaction with other medicine

Monoamine oxidase inhibitors (MAO inhibitors): xylemetazoline can potentiate the action of monoamine oxidase inhibitors and induce hypertensive crises. Do not use xyletazolin in patients who have been taking or taking MAO inhibitors in the last two weeks. Three- and tetracyclic antidepressants: with the simultaneous use of three- or tetracyclic antidepressants and sympathomimetic drugs, it is possible to enhance the sympathomimetic effect of xylematezolin, so the simultaneous use of such tools is not recommended. Application along with beta blockers can cause bronchial spasm or lower blood pressure.

Precautions

The drug should not be used for more than 5 days without a break. Prolonged or excessive treatment with xylometazoline can cause secondary swelling of the nasal mucosa with a risk of chronic rhinitis and/or nasal mucosa atrophy. The drug, like other sympathomimetics, should be prescribed with caution to patients who have strong reactions to adrenerous agents, manifested in the form of insomnia, dizziness, trembling, cardiac arrhythmia or increased blood pressure. Do not exceed the recommended dose of the drug, especially in the treatment of children and elderly. It should be carefully prescribed to patients with cardiovascular diseases, arterial hypertension, patients with diabetes, with thyroid hyperfunction, feochromocytemia, prostate hypertrophy. Do not use patients who received concomitant treatment with MAO inhibitors and within 2 weeks after their cessation. Patients with prolonged QT syndrome who receive xylometazoline may have an increased risk of serious ventricular arrhythmias. As a therapy of the first line, it is recommended to wash the nose with a physiological solution. Otrivin with menthol and eucalyptus can be used as a second-line drug. The drug contains benzalconium chloride (0.014 mg of benzalconium chloride per dose of 0.14 ml, which is equivalent to 0.100 mg / ml), which can cause swelling of the nasal mucosa, especially in the case of prolonged use. When using sympathomimetic drugs, isolated cases of the development of posterior reverse encephalopathy syndrome (PRESS) / reverse cerebral Vasoconstriction syndrome (RCVS) were reported. Symptoms reported included sudden severe headache, nausea, vomiting, and vision impairment. In most cases, the weakening or disappearance of symptoms occurred within a few days of appropriate treatment. You should immediately stop using the drug and see a doctor if signs/symptoms PRES/RCVS develop. The drug can not be used during pregnancy due to the potential vasoconstrictive effect. Evidence of any unwanted effect on the baby is absent. It is not known whether xylometazoline is excreted in breast milk, so caution is necessary, and the drug during breastfeeding should be used only as prescribed by a doctor. When breastfeeding, prolonged use should be avoided, given the risk of undesirable phenomena in the baby (taicidia, excitation, increased pressure). The drug is not used in children under 12 years of age.

What side effects may I notice from this medicine?

The drug is well tolerated, including patients with sensitive mucous membrane, and does not reduce mucocyliar function. The results of laboratory tests showed that xylometazoline reduces the infectious activity of human rhinovirus associated with a common cold. Excessive local use of xylometasolin hydrochloride or its accidental ingestion can lead to pronounced dizziness, sweating, a significant decrease in body temperature, headache, bradycardia, less tachycardia, arterial hypertension, respiratory suppression, coma and court. Increased blood pressure can change to reduced blood pressure. Violation of consciousness can be a sign of severe obvious intoxication. Young children are more sensitive to toxic effects than adults. At home, you can take activated charcoal as an emergency measure for hospitalization. In the case of severe intoxication, the patient should be hospitalized, urgent symptomatic treatment is carried out under medical supervision. Medical care should include monitoring the patient's condition for several hours. In the case of a severe overdose accompanied by cardiac arrest, resuscitation measures should last at least 1 hour.
